According to wikipedia, one of the filming locations of The Reichenbach Fall was the Newport Cemetery. The official name of the cemetery is actually Newport and St. Woolos new cemetery (and again, according to wikipedia, this cemetery was also used as filming location for Dr. Who). Just a random fact - this cemetery is only 14 miles apart from Cardiff, where Steven Moffat does live.
The question is, where is Sherlock’s grave? Here are John and Mrs. Hudson at his grave:
The building in the background is the West Chapel and I used it as orientation point. You can also see the tiny road behind their backs.

second question is, where was actually Sherlock while looking at John?

Just for the information, the grave’s distance from Sherlock was aprox. 40 meters. I’m not sure if he could hear John’s words, but he sure saw him cry.

and we’ll never know how Sherlock survived.

I know how Sherlock feels after Reichenbach.
He doesn’t look hurt most of the time, because most of the time he is not. He pushed John in a far away corner of his mind palace and started ignoring him, working restlessly to bring Moriarty’s net down, barely sleeping at all.
He never thinks about him, and when he does it’s brief and emotionless, before he hurriedly pushes him away once again.
He’s so tired when he goes to sleep that he doesn’t have time to think nor dream, and when he gets up he restarts the day all over again.
But there is a moment, every once in a while, after managing to strike a good blow to the criminal web, when everything slows down and he gets some time to think.
And when he does he takes his mobile and reads some of John’s old messages, all these small things he might not hear again, like “You are fantastic, but you could still get the milk.” or even sillier things.
That’s when he breaks down, when he goes fishing for John in his mind but finds out he has suppressed him for so long he doesn’t even remember the sound of his voice anymore.
That’s when he manages to cry, and understands that perhaps he’s not a sociopath at all, because he is broken, and lonely, and it hurts so much that he, for once, feels like a human being.

So, this was it. The last day of filming. Well, probably it wouldn’t be the last, there would be things to adjust, shots to redo and maybe even dialogs to dub, so this isn’t the end, not really.
But still the scene they have to film is going to be the worst one he has ever made with Benedict. It’s not difficult per se, just Sherlock jumping, the puppet hitting the pavement, then Benedict lying there and whatnot. He, as John Watson, only had to approach the supposedly dead body and check it’s pulse, then sit there shocked.
He could do it, right?
But the thought of watching Benedict jump from there, even if only fictionally was horrible. And when the actual scene came in, when the camera started filming and they where on the bloody phone… Yes, the call was real.
They were talking, and Benedict (a man who has become a really good friend to him, and who he really likes) with that damn voice tone of his was so convincing…
It all looked too real to him.
That’s why after the jump, even after the stop to remove the giant mattress that caught Benedict and after he had been covered in fake blood and placed on the pavement, it was easy for Martin to express such a feeling of loss and apathetic pain, as if his heart had been tore away from his own chest.
And that’s why when the shooting ended he ran up to Benedict and hugged his tall friend as if he really just came back from the land of the deads, clinging on him for dear life, ignoring that he himself was getting covered in fake blood.
Because he’d never want to see something like that happen to a friend of his, not even if everything’s fake and safe, and he certainly doesn’t want to see it happen ever again.

EDIT: Now I know how they filmed the scene, and I know for sure how little sense this makes, but still here it is.
